Quick question for coding LBBB and RBBB, Specifically for cardiology board exam.

for RBBB, criteria mentions rSR’ in V1, with slurred S wave in 1,avL, V5,V6.

for LBBB, prominent S waves in V1, with Monophasic R waves in 1,avL, V5,V6.

my question is, I have been practicing with ECG source, and there have been several questions where for a RBBB, V5 and V6 did not have a slurred S wave, but 1 and aVL did, so it coded the correct answer as RBBB still. same thing with LBBB, no monophasic R in V5, V6, but they were present in 1, aVL.

what is the correct coding method. Do you need to have the changes in V5 and V6 for the bundle branches, or are the respective changes in 1 and aVL sufficient.

also, finally, a silly question. What lead do you guys prefer to measure your QRS duration in? Is there one that is preferable to other.

thanks in advance!

ECG is frustrating. Guidelines vary as do interpretations from various sources.
