college grades

If I'm not mistaken, the opthalmology residency process requires submitting undergraduate grades. Why do they ask for these, and do they play any important role in the resident selection process? I'd imagine most med school applicants had uniformly good grades in college, anyway.
 
Do college grades matter if they were sufficiently long ago? For example I am interested in opthalmology, have poor college grades, but did some graduate work with excellent grades before entering medical school. I know its competitive out there, should I look into something else. College was so long ago.
 
Would an undergraduate 2.8 be a dealbreaker?
