Columbia App Question

I just submitted my columbia application and when I received confirmation of my submission I was told to mail in a photo using the photo form. I CANNOT find this form!! Anyone else find it? Thanks!

I called them up a few weeks ago, they said there isn't a form, just mail it in with your name and social security number. The lady I talked to seemed pretty annoyed at having to answer that question, I guess she had been asked it many times already. 😛
