Columbia MSTP - need GRE?

Hi, so I haven't taken the GRE b/c none of the med schools seem to need them, but Columbia has a spot to fill out GRE scores... does anyone know if it's mandatory to take the GRE for Columbia's MSTP program?

For that matter, does any school require the GREs for MD/PhD applicants who are looking to get a PhD in the basic biomedical sciences? (e.g., not ethics and/or engineering)
 
University of Florida requires a GRE score from all MD/PhD applicants.

So far, they're the only one that I looked at with a mandatory GRE.
 
Does anyone know the "code" for the biology subject GRE? I took it and thought I would include it on my app since I got a good score, but I don't know what goes in the code field to let them know which subject test I took.
