For those who plan on doing an allopathic IM res. with just the COMLEX do they need to have taken the USMLE for applying to fellowships (including the competitive ones) in general?

No; they primarily want you to be have a license. If they're willing to look at you as a DO, I don't think the subject will come up. Don't believe board scores even come up in fellowship applications.

I don't think it matters at all. The key things are research, letters of recommendation, and where you trained (in that order).
