Just took it two weeks ago and got nervous. For two patients I just said you have this diagnosis and didn't really expand upon it. They weren't crazy medical jargon, and I think they were mostly self-explanatory, but I do wish I went into depth about them. I still did give them a good plan.
On two other patients I said that for these reasons I don't think you have this diagnosis and I'm going to do this other step in one patient or send you to a specialist in another patient. Besides that I feel confident. Any advice? Should I stop worrying?

you don't have to expand on your diagnosis. You just have to tell them what you think it is and what your plan is. Definitely stop worrying. I was freaking out too because I forgot to wash/sanitize my hands for few. My last patient of the day had to remind me to sanitize my hands. I was also nervous regarding the fact that I left nearly every room when they announce the 2 min warning. I passed. You will be fine.

I was wondering how many mistakes can be made in the SOAP note section? I am a bit worried (still have not received my score yet) over whether or not my SOAP notes will make the cut. My objective, assessment, and plan were more or less always complete but when I finally got to my subjective part of the note I would be rushed for time. Can't remember if my actual HPI had any relevant information besides two or three sentences but i always included the rest of the subjective portion (ROS, FH, PMH, SocHX, SurgHX, medications, allergies) Does anyone have any input on the SOAP note section or words of wisdom so I can maybe stop freaking out so much about this exam. Any help would be appreciated.
