Before comitting to a school, how do I proceed with schools who haven't given me any decision (ghost) and those to whom I have submitted a primary but not a secondary?
Do I need to reach out to all these schools and formally ask for a withdrawal before I commit to the school?

You do not need to reach out if you never got an interview or did not fill out a secondary.
You can PTE (or CTE) without notifying other schools. AMCAS will do that for you at the other schools where you have interviewed.
It is polite to withdraw from waitlists if you have made up your mind though!
