Community programs affiliated with a prestigious university?

I've had this question for a while after researching residency programs. There are a number of community programs that are affiliated with prestigious universities that interest me.

UCSF has its UCSF program for IM and a UCSF Fresno program. What is the difference between UCSF Fresno compared to UCSF and do people distinguish the two programs? How about fellowships? How would a program like UCSF Fresno compare to a purely community hospital program does the UCSF name help in anyway?

There are many other community hospitals affiliated with top universities like this: Mount Auburn, St Luke's Roosevelts

I know that university programs are considered better for obtaining competitive fellowship positions but how does a top university affiliated community program compare?

UCSF Fresno is in a different universe than UCSF. Same with Hopkins Bayview vs. Hopkins Osler etc etc. Laypeople may be impressed, but anyone giving out fellowship positions won't be.

Every year people rank these big name affiliates (which are just community hospitals) over solid actual University programs. Don't let your insecurity get the best of you and stop getting so caught up in "prestige". Find a place that will make you happy.

Putting Bayview in the same category as UCSF Fresno is kind of ridiculous. Is it as "good" as Osler? No, but it's probably the best community (IM) program in the country and has very strong ties to the mothership with shared faculty and rotations. It will set you up nicely for most fellowship opportunities.

UCSF Fresno OTOH is probably the 6th or 7th best community (again...IM) program in CA and has essentially no connection with UCSF.

In most cases, these affiliations are for the benefit of the faculty, not for residents. It helps them recruit to "lesser" places if they get a faculty appointment as well.
