Comprehensive and updated list of which schools do grade replacement/forgiveness etc

Hey guys,

So I'm a long-time skulker, but I wanted to see if I could get help putting together an updated list of which schools do grade replacement, focus solely/more on the last 45 credits, or only average the two grades. I got this from an old thread, need help verifying and adding to it. Thanks!

Schools that take the NEW grade: Only one I'm sure of is Iowa because I just checked
Western
Ohio
Michigan
Mississippi
Louisiana - if old grade over 6 years old
Kansas
Colorado
Oregon (for prereq GPA)
Washington (for prereq GPA)
Wisconsin
Iowa

Schools that averages both grades:

Tufts
Purdue
Oregon (for cGPA)
Washington (for last 45 GPA)
Minnesota (for last 45 GPA; if the first attempt was less than three years from application submission)
Louisiana (for last 45 GPA)
Missouri
Oklahoma State
TAMU
University of Florida
University of Tennessee
Virginia-Maryland

Focus on last 45:


Extra:
-Kansas looks at last 45 lot
-UMN looks at prereq GPA and last 45 only
-Iowa uses last 45 and science only
-Michigan cares a lot about last 45, doesn't look at cGPA at all
-Mississippi looks at last 45
-Colorado does not use cGPA; looks at trends
- TAMU participates in the Texas Fresh Start Program which results in grade replacement.

Michigan State is the last 3 semesters (36 credits minimum undergrad / 18 credits minimum graduate).

Also, when I met with admissions there, she said they take the highest grade achieved in the required courses, even if that isn’t the most recent grade.
