Contact info in AMCAS work/activities

Do we need to put down contact info for each of our EC's? For instance, I have no idea who I would put down as a contact for a scholarship I recieved as an entering freshman.

any help would be great

also, is the country, state and city for the organization or for where the activity took place?

It's just generally a good idea-- keep you from looking suspicious.

If your scholarship was given by your university, I would write "XYZ University, Office of Financial Aid." City and state would be the city and state of your Uni.

If your scholarship was given by an outside organization/corporation, I would write "ABC Corporation, Office of Public Relations." City and state would be the home base of the organization/corporation.
