CORD LOR Help!

Hi,

I am curious if filling out the CORD application form counts as an LOR or if I need my Letter writer to fill out this form and write an additional letter? I'd really appreciate anyone's reply! From an old 2005 post I inferred that the CORD form it's self is sufficient, but I am looking for conformation.

Also, at the end of the form there is a signature line and beneath it and it states, "Student has waived right to see this letter." Does my Letter Writer sign this if I have waved my right to see the letter, and conversely leave it blank if I didn't? Again, any insight here will be deeply appreciated. I'm a nontraditional candidate and really have no one to ask about this. 😕

the cord LOR form is it. traditionally the evaluator will fill out the entire form each time you do an EM rotation. actually they should have it and, in theory you should never see it.

So when asking for a SLOR, do I need to provide each of my letter writers with a copy of the CORD pdf plus my CV? Or, will they have the SLOR form already?

They should probably already know what the form is and have filled it out many times before. FYI if they have no idea about it you may want to reconsider having them write a letter.

they should have the form but here's what I did:
1. provide them with the form
2. your cv
3. other LOR's/thank you letters from docs, pts, nurses...etc
don't give any pdf files. print everything out. the easier you make it on them to write you a letter, the better.

Am I supposed to be getting LORs from nurses/patients/etc? What are thank you letters in terms of being a med student?

you don't "officially request them" but if some nurse jots down an email to the PD that they really like you to stay in the program or a pt sends in a thank you note/card to the administration...all of those little things help.I had a couple that were sent to me by the nurse manager, which I happily re distributed in my interview packet. believe me nurses talk and PD's know which ones to listen to. get separate from the rest of the interview pack, that's the whole point of this
