Hi.

I saw this inflammatory cells dissecting the muscular layer of colonic mucosa in the context of a villous adenoma.
This may be a silly question, but I want to know if this change has a significant association, and how can I call it. I looked at a couple of books, and google but cannot find it.

First off your picture is crap. Second off that doesn't look like "muscular layer of colonic mucosa" (most people call that the muscularis mucosa), it's too thick unless this is the TVA stalk. It's probably the muscularis propria. Third that's nothing. If there's a dense lymphocytic infiltrate you can suggest microsatellite instability, but what you're looking at is nothing. Is this your first week signing out or what?
 
Dude, I think those are vascular spaces. The inflammatory cells are quite possibly in the vascular spaces, which is where they often are.

Second, are you sure that's muscle?

Thirdly, I'm pretty sure you missed an acute leukemia and the patient is toast.
 
yeah, those are vascular spaces full o' eos running through what appears to be smooth muscle. i wouldn't get too excited about eos unless they are extravasating into the mucosa or if there is an eosinophilic colitis. in summary and based on this EXTREMELY limited field - it's nothing. i wouldn't even get into the parasite/allergic/drug rxn differential.

If you're a med student it's totally excusable. Sorry. Good luck. Don't use oil on surgical pathology, stay high and dry.
 
That picture looks exactly like the quality of image I had on the AP/CP boards?

Are you sure you dont submit questions for the ABP?
 
your pic is crap as someone said. This finding could be totally meaningless as:
1.) inflammation is common in ANY type of polyp
2.) eos are a normal component of the lamina propria in the bowel

your histology looks like crap too not just the pic, yikes.
 
The pic just looks like someone snapped it through a microscope ocular with a cell phone.
