So I'm really heavily involved in an activity, where we organize and teach test prep classes for low-income students to help them get to college. I think it falls under both teaching and nonmed community service, but how does this actually work out on the application? Do people just cross-list these things and then clarify the details?

You choose the one category that helps balance your application best, making it clear in the narrative that the activity covers the other designation as well. If you have nothing else for Community Service/nonclinical, I'd put it there.
