With the exception of graded motor imagery, bisphosphonates are the only therapy (considering mental health, standard physical therapy, interventions, any other drug) that is 1A+ evidence (using the modified Guyatt evidence assesment grid.)

Given this fact, how many actually use bisphosphonates for CRPS?

They are kinda dangerous drugs, but they seem to work for CRPS. It presents a cunundrum, right?

oral maxillofacial surgeon friend is seeing a spike in jaw necrosis cases, and they're horrible outcomes postop.

They almost always happen s/p some jaw procedure (previous surgery). Also, I think there are other risk factors as well that could be mitigated or avoided.

But still, is it worth it to give someone in horrible pain the only drug that seems to have good data to support it - given the risks of the drug?

Renal failure happens with these drugs. But again, perhaps this risk can be minimized.
