Cumulative GPA question

So, to put a long story short, my first 3-4 years of college were sloppy. I have one degree from a community college with a GPA of ~3.3.

I did one year at another college and needed to drop classes and ended with a D in one.

Fast forward to my current schooling , I am sitting on a 3.91 GPA with A's and A-'s, as well as one B after 2 years. I plan to finish this coming spring with a GPA no less than what it is now.

My question is, when schools look at the cumulative GPA, will they look at my last 3 years because they were my most recent grades or will they look at everything and hold my past against me?

Also, the class that I got a D in was retaken at my current college, and it ended up being an A. Will this overwrite my previous grade of a D if they DO factor every grade in?

DO schools do full grade replacement for the most recent course retake. the course retake needs to have equal or greater units than the course being replaced. the exception is Texas DO schools that only average your new score with your old score(s).

Do you happen to have any idea how they look at grades that were at different colleges, especially if the most recent college showed much better grades?

EDIT: I mean, if my first and second college had a GPA around 3.2, and my GPA at my most recent college is 3.9+, will they even look at he 3.2 GPA? Will they give the benefit of a doubt?

I read on a few schools' websites that they look at the "last 120 credit hours" more closely than other credit hours....not all schools said that, but it's something....
