Curious question about glaucoma

I was in ophthalmology clinic and a 54 yo patient came in for review of his right eye that was apparently blind because of glaucoma. The doctor let me have a look through the slit lamp and told me it's something I am probably never going to see again. So I had a look and just saw a complete bright spot at the back where the optic nerve emerges with no discernible cup/disc.

What does that mean? Is that supposed to be a cup-to-disc of 1? Does that make sense?
 
did you use a lens when at the slit lamp?

Hahaha we did this really awkward thing where she is like "okay I am going to hold this lens really still and bend my neck. You look through the slit lamp."

It was quite cool since it was the first time I ever looked through one.
 
Nice. I remember those days.

The optic nerve has been completely "cupped out." I've never seen someone write a cup to disc ratio of 1 (but that's essentially what it is). A lot of people write, "Out To Rim" or OTR. If you go into ophtho, you'll see it many times.

Haha, I love that this is so many people's memory of first looking through the slit lamp. When I did an away rotation at a place that had a side viewing port on some of their slit lamps my head almost exploded.

Thanks for the clarification. Maybe what she meant is that I will never be smart enough to get into ophtho to see it again.
