D.O. letter of recommendation another question

So I'm shadowing a D.O. physician this week in conjunction to asking for a letter of recommendation on my behalf. I am planning on handing a hard copy of cover statement, c.v. along with a personal statement.

In filling out the letter of recommendation through my school's office, does this physician need to know anything else before hand? I'm thinking that this is all they need to know to fill out the paper work but I don't want to be amiss about anything I need to do.

Also, what should I tell this physician for the deadline to be due? I want it obviously to be asap as apps are going on right now, and the sooner the better, but AACOMAS is still processing my primary application (And I sent that in the 1st week of September). So I haven't had any screened apps telling me the deadlines that are due next.

Any opinions would be appreciated. Thanks.

hey there. i had my personal statement and cv with me when i asked my DO doc to write a LOR. it seems as though those 2 things in addition to how you handle yourself during your shadowing experience will be enough for the doc to go on--to be sure you could always ask--(they usually say they need a statemtn and cv). i also included a phone number and e-mail in case the doc had questions.
i set my deadline about 2.5 weeks after i asked the doc, but also made sure to ask the doc if this was a good timeframe. hope this helps!
