Dental assistant Job in Texas

Hey everyone,

I got a dental assistant job. However, I am confuse on what exactly do I have turn in for my application. I completed a course online and received a certification on June 2012. Do I just send that certification along with CPR card, social security card, and several other basic stuff. Which I got them all down, but do I also need a DANB Certified dental exam/form to complete my application?

I am quite confused.
