Saw this story. Could anyone explain how exactly this happened and how it could be prevented?

need to turn down or the oxygen to a safe level if using any ignition source (e.g. laser, electrosurg)... this is a well known risk in the medical field. If you ever work in the OR they'll ask what the fire risk of the case is and if you're using any lasers, etc. People die this way under general anesthesia because it can become essentially a blowtorch in the lungs.

$15000 is small stuff for a lawsuit. I'm a little surprised.

Yea, they got off real cheap and easy. Hopefully it's not too publicized and if so, they better have a good online reputation management company to drive those negative articles down page one and replace them with glowing articles (or rebrand altogether) or just DMCA the hell out of the articles. One place here killed a boy and for the longest time it was known as the place that killed the boy. Bad for their business, good for ours.
