Is a letterhead absolutely necessary for a dentist LOR?

The letter has the handwritten signature + credentials + contact number, but it's not on an official letterhead. My dentist is a busy guy so I don't want to bother him too much by nagging about little details...

No, it's not a requirement - just make sure all contact information is on the letter.

That's taken from ADEA. So it's not absolutely necessary, although it's recommended.
