I have already applied to a number of programs by assigning 2 LoRs that are uploaded/ready to be sent.

I haven't designated by third letter writer yet because I am a reapplicant and I am waiting for the updated letter to be received/scanned by ECFMG - and am worried that the old letter will be updated and sent automatically if I do (which I do not want).

Is this causing a problem with my current application - do programs only think I have 2 letters to offer?

Short answer: No.
Long answer: Nooooooooooooooo.

We can't see who is "designated". We can only see who is received. If I looked at your application today, I'd see two letters. I'd have no idea if you had designated any further letters for my program. All I'd see is 2.

So, does that mean they don't know I have another letter coming and they might reject me without waiting for my last letter?

I have 3 letters already uploaded but 1 is chair of med, 1 is clinical, and 1 is research. i am still waiting for my 2nd clinical letter to be uploaded but didn't want to wait for it to submit my application. Will they look at this and see wow, this person couldn't get 2 clinical letters and submitted this research one instead?

How can i let them know i have another clinical one on the way? Thanks.

When You refer to the Chaiir of Med letter, is that the letter that many preliminary medicine internship programs require? If so, I was under the impression that it was like a mini "Deans Letter". Where does one obtain this type of letter? From the Deans office or from the Int Med Dept?

i got mine from the chair of medicine/head of medicine from my hospital/med school. Does anyone know the answer to my question above?

Did you get this letter specifically for your prelim medicine internship year requirement, or for an internal medicine residency requirement?

i got it for internal med categ, not prelim. not applying for prelim.

Thanks for all your responses to my questions. Just one more; Do you think your program requires the "Chair Letter" if you are just applying there for a preliminary medicine internship year?

You are correct. I will see three letters, and cannot tell if there is a fourth letter coming, nor whom it will be written by. Some programs may consider your app complete and review it. There really isn't anything you can do about this. You could start emailing all the programs to try and explain this, but that could easily do more harm than good.

These letters come from the IM dept.

Some prelim programs require an IM dept letter. Some do not.

If the IM residency program website says that a Chair letter is required, can you assume that it refers to both the categorical IM residency as well as the Preliminary Med internship program? Or should you not assume, and call each program to see if the requirement of a Chair letter applies only for the Categorical IM Residency program?

AProgDirector, sir, what if you fail to designate your dept letter as the "chair letter" or "department letter," listing only the names of the two people composing the letter along with "department of medicine." I think programs will have to begin reading this to realize it's the chair's letter. Will this be a problem? I'm afraid some may consider mine incomplete without reviewing it, not realizing the dept letter is in front of them.

The easiest thing to do is assume that the chair letter is needed for both -- this is no work for you, assuming you have a chair letter. The harder thing to do is contact each program and clarify.

Not a problem. Programs will review your letters. Many chair letters are listed like this.
