Usually, you need to be admitted to MD first and then are considered for the MBA etc. Same pool for all MD candidates (MD/PhD is a different story and a different pool and a very different set of criteria for admission).

Usually this is why MD/MBA or mph programs don't really have a separate criteria for assessment. It is much easier for one to adopt the MPH or MBA as a dual degree than a phd which offers incentive of full funding that is harder to receive and does require a different level of scrutiny.
It depends on the school... some arrange the MBA as a one-year, full-time program after M2 or M3 year and so you apply while in med school with the intention of taking a year off from med school to do the other degree.
