yes, especially if you failed one of them. Step 3 is considered of little value. Now, they tend not to be "deal breakers", but they are considered in my experience.
Very high scores (define as you wish...) are definitely noticed as well.

Yes indeed. They don't count for everything obviously, but a very low score or a very high score sends a message.

What is the most important factor in fellowship application?
As with most jobs, it is your interview impression. Letters of rec matter a lot for many fellowships as well. Essay, "activities" are less important. Research is very important for some, but not most places and fields.
Don't think anyone can answer that. I look at where you went to school and residency first (probably because it's easy to find). I look at board scores next, these are a big deal to me. My fellowship program is evaluated on how well our trainees do on the boards. I read your letters. You can tell when people think you are good and when you are not (usually). I look at you PS only to assess your ability to write and that you don't seem to be a narcissist. Your publications are only important if they are real and there are more than one or two.
