Do residency apps ask if you took both Comlex + USMLE?

If you are talking about ACGME and one of the programs that are part
of ERAS ( http://www.aamc.org/eras/resource/eassoc/programs.htm )
then your answer is yes...
Also on your ERAS application you can only transmit your USMLE scores
electronically. If you have taken COMLEX you have to make sure that
your school scans your results and send it along with your
transcript. Most residency programs know about this so this is not a
big problem or disadvantage. I hear that ERAS is working on
modifying their program so that you can send your COMLEX scores
electronically (directly from NBOME) as well.

If I were to request that my school not release my USMLE score, is there any way that residency programs could get ahold of it, or even find out that I took the test? I take the USMLE next week...

If you apply to one of the ERAS programs (see above post) you will be asked if you have ever taken the USMLE once you click yes they will obtain your scores electronically from NBME.
