For those of you who have already requested LORs from your attendings, did you give them anything else other than the CV, personal statement, and ERAS cover sheet? I am considering including a copy of their evaluation of me from 3rd year too...

For those of you who have already requested LORs from your attendings, did you give them anything else other than the CV, personal statement, and ERAS cover sheet? I am considering including a copy of their evaluation of me from 3rd year too...

I think that's a nice edition to the package. You already know to this but for completeness sake, don't forget the disclaimer that I only gave you this evaluation for convenience and not that you forgot how I did. Saying something like that just shows you are thoughtful and not insulting their intelligence if they happen to be one of those prudish types.
