When applying to optometry jobs, does one need to submit graduate and undergraduate transcripts for:

A. government

B. corporate office (bausch and laum)

C. commerical/private employment

What do optometry jobs usually want document-wise?

You should provide a CV (curriculum vitae) including a list of references and a cover letter. Rarely will they ask for optometry school transcripts.
 
Are you refering to applying for jobs when you graduate Optometry school or jobs working in an Optometric practice as a tech/asst? If you are an Optometrist, most places will ask for proof of a state license, proof of graduation from Optometry school (usually a copy of your diploma or a legible photo), and your CV/Resume. If you are applying for a job as a tech/asst., they will probably only ask you to fill out an application and a resume with references. Hope this answers your question.
