In order to practice medicine in Ontario, you need to be licensed by the
College of Physicians and Surgeons of Ontario.
The requirements for independent licensure are as follows:
1) A medical degree from an accredited Canadian or US medical school or from an acceptable medical school listed in the World Directory of Medical Schools.
2) Successful completion of Parts 1 and 2 of the Medical Council of Canada Qualifying Examination.
3) Certification by examination by either the Royal College of Physicians and Surgeons of Canada (RCPSC) or the College of Family Physicians of Canada (CFPC).
4) Completion in Canada of one year of postgraduate training or active medical practice, or completion of a full clinical clerkship at an accredited Canadian medical school.
5) Canadian Citizenship or permanent resident status.
In the case of a physician shortage in a particular field/geographical area, the College may also choose (at its discretion) to permit physicians - who are otherwise not eligible - to register via the
third or
fourth pathways.
If you wish to attempt this route, you will need to contact the CPSO to determine whether your DO degree (I assume from your asking about COMLEX) satisfies the requirement for a "US medical degree". (Read through the third pathway links for the details on this route to licensure).

It's also important to note that these are merely the requirements to be licensed by the College; in most cases, the individual hospital/health region will ALSO have requirements (for example, passes at the MCCQEs, ACLS certification up to date and so forth).
