does volunteering need documentation?

so i just remember i did about 4-5months of volunteering at a hospital about 1-2years ago. do i need documentation? i actually dont remember the exact hours because they were random.

im not applying this year, but when i do, do i need to provide documentation? if not, what if i dont remember and just happen to exaggerate on my application?
 
You don't need "documentation" per se but its good to remember the number of hours you did and include those on AMCAS. I help run a clinical volunteering program, and we provide letters of rec upon request, as well as verification of how many hours were completed. Maybe you can get something similar?
 
You don't need documentation unless required for a committee letter, but you will need to provide contact information. Why not call the hospital volunteering office and see if they have a record of the hours you were there?

Agreed. In my limited experience, hospital volunteer offices keep records of hours worked. After 1-2 yrs, they may have discarded them (if they're paper records), but it's worth a try asking.
