Continue something you're good at and/or passionate about to the point where your skill is significant nationally or internationally.

If you're asking what can be done from scratch in a year, your options are way more limited. First thing that comes to my mind is entrepreneurship. Not unheard of to grow a company from $0 to $1M+ annual revenue in a year, but good luck with that lol
 
Do whatever allows you to have the best possible grades and MCAT. Enjoying what you're doing is always a plus. Unless you're doing something genuinely unique, like being a world class Olympic athlete, Fortune 500 CEO, or cure AIDs, it's all going to be similar to other applicants.
 
Stats aside, what would you recommend for standing out in terms of ECs in a year or so for the Top10???
Service to others less fortunate than yourself, and lots of productive research.
Volunteering hours > 300 for clinical and non-clinical each.
