you could write "dance instructor" or "fitness instructor" or even "small business owner" or something. but i don't know, i dance pretty seriously and put it on my AMCAS and got asked about it at basically every interview. so, be prepared for interviewers to ask for clarification-- "what kind of dance?" "oh, that sounds cool, what sort of fitness classes do you teach?" etc.

I would put small business start-up/ownership under Leadership. Maybe you're not leading anyone, but you took some initiative, looked into the financial/legal/tax aspects, advertising yourself, and hopefully made some money.
