Important to top schools: Everything (both research and volunteering).

I wouldn't focus on just filling in blanks with fluff, you have to be able to actually talk about them intelligently during an interview.
 
Absolutely not. 15 is maximum, I'm thinking plenty of people don't fill out all 15.
 
also is deans list even worth mentioning?

i really dont know what to be involved in this year... either a new research lab, because my old one got really boring...or more involved in community...
 
is it worth getting into a new lab now, if your applying june 2010?

I didn't.

The very top tier med schools probably tilt toward research, as they tend to be more research and academia oriented. But you're going to need everything and the kitchen sink to get in, so it's not a question of 'or' you need research, volunteering, and much more.

On the dean's list, I'd put it, but not as it's own entry. I think I summed up all my dean's lists, a departmental honors, other little awards, etc in one entry.
