Failed CS !!! WTF

I just recieved my score and it says I failed the data gathering part of CS !!! I attend a US Medical school and have honored most of my rotations and have been told multiple times I am great in clinical settings ... Why did I fail? what should I do now?

Please advice

Reschedule to repeat it. Read Step 2 CS. Practice each scenario with another student. That book goes through pretty extensive "what you should do" and "what questions should you ask" for each complaint. It then give sample notes that you should be writing. It also gives advice designed for people who would otherwise fail.

Its a contrived exam. Learn how to pass it, then pass it.
 
Your mistake was to treat it like a real patient encounter. Passing this exam has little to do with your actual clinical skills, and almost nothing to do with your diagnostic acumen.

Agree with the above poster. Figure out what they're looking for and do it. I guarantee you it's something ridiculous, that YOU aren't doing because YOU don't need to in order to figure out the problem. But, again, the test is not about your diagnostic skills, it's about getting the SP to check off boxes.

Keep this in mind on your retake: the SP can't see your note, and can't give you credit for getting the right answer with a few key clues. He or she can only give you credit if you do what it "should" require to get the answer.

Likewise, the MD who sees your note can't see any part of the actual patient encounter. So the note needs to stand alone and support your conclusions, document what you discussed, your findings, plan, etc.

🙄 you've gotten past every other obstacle, you'll get past this one.

One last piece of advice: take the test in a different location next time. I don't know if it matters, but it can't hurt.
 
Inter-rater variability is always higher than intra-rater variability. And I'd bet inter-center variability is higher than intra-center variability. I don't think they flag you as a re-tester, and if you go someplace else, there's zero chance of getting any of the same SP's. At the same center, there's at least the potential that you will. Yeah, they see 14 or so students every day. But I remember patients I've seen before, even though I see tons of them every week. Why would the SP's be different?

I think they just pick somebody to fail each session, because somebody has to fail to justify giving the test. But whether you drink the Kool-aid about the value of this one or not, you still have to pass it.
 
I thought you were looking for people to "please advice" re: your chances of getting into dental school in July. Did you end up getting into med school instead, completing your first 2 years, and taking the CS w/ results back...in 6 months? Right.

I had 2 questions:

1- For female pts when you are doing the pulmonary and CV exam, do you untie them completely? meaning you are examing above and below the bra or you just untie is a bit and still keep their modesty?

2- When do you drape the pt? soon after you get in and introdue yourself and before start asking questions?

Please advice!

Every patient gets a heart and lung exam no matter what the CC. I would just go ahead and untie the gown, apologize but explain it's necessary then proceed with the exam. Then let them re-tie at the end to save time...

For the draping, what I did was come in, throw my gloves on as I was introducing myself (timesaver) then immediately drape the patient. That way none of the above got forgotten.

Also, don't forget that you need to ask EVERYONE about:
Allergies, PMH, PSH (OB hx if applicable), Social and Sexual history and offer counseling. I suspect this is an easy place to miss enough points to drag a pass to a fail.
 
Sorry this happened. They are looking for specific behavior patterns and keywords. You have to perform in such a way as to give the grader a opportunity to check off their checklist. I would practice using First Aid. Either do this with a partner, or by yourself. You have to imagine that you are in the actual exam while going through these practice scenarios.
