Help!!! I failed in data gathering and I'm not sure why. I'm sure I asked almost all of the questions in HPI, PMH, SurgH, Meds, Allergies, FH, SH and Sexual H. My ROS was focused on the cc and not a general head to toe ROS. I passed everything except data gathering. It was off the chart low; nothing but an asterisk. My pt note and questioning skills were good, so I don’t understand. Has anyone had a similar experience with a pass on the 2nd attempt?

Sorry to hear about your experience. I know an anesthesia resident who claims to have had a similar experience to you, and passing the second time by conducting a more thorough ROS.
 
Its painful ...its ok focus on future...Try to analyse well on your exam experience and try not to repeat mistakes. Everyone commit mistakes but see that mistakes are less

Hello friend, am sorry to hear about that result. Its difficult to forget that. But in life failures are not uncommon but you need to bounce back again and show that you have your unique way of dealing. Past is past just focus on future but learn from past and see that not to repeat in future. This theory applies to not only exams but to life.

Some times passing cs is pure luck according to exam given students. But better we should not depend on luck. I am sure you might have worked very hard. Failing in data gathering means may be you know what to ask the patient but in exam tension at that moment you might have missed some basic stuff.

The only solution is practice practice practice practice practice practice


practice makes you feel free while dealing with SP in exam and you can lessen your anxiety so that you can ask all questions required to the patient. One more thing i think you know it, give time if SP is trying to tell you something..

Hello,

Don't let CS bring you down. You need to get right back up and start going over CS cases again.

First Aid CS is all you need to pass! The trick to passing CS is to have your Mnemonics ready for each case. The second trick is to have a consistence flow for each pts. I have trained many friends to pass the cs! Ex.

(if you keep this flow, you'll pass!! not only that but show sympathy too)

Hello Mr. Jones! How are you? My name is Dr. John Doe and I am the attending doctor here today and will be taking care of you? Before I start, is it okay if I drape you first? (remember to say thank you for everything and ask, be polite before doing anything)

Before we start, is it okay if I sit down, and take notes while we talk? Thank you Mr. Jones.

So, Mr. Jones, What brings you here today? ...
Aw, I'm sorry to hear about your pain. Don't worry, I'm here. Lets work together to find out what the cause might be.

*Here, you should have your mnemonics ready and written down, and just follow your mnemonics."

Once your data gathering is done, do PAM HUGS FOSS WADE Last!! Once that is done.. say something like..
"Mr. Jones let me recap everything of what you just told me..."
"Mr. Jones is there anything else you would like to add that I haven't ask you yet?" -->very important, because a lot of time pts will tell you other missing information!!

Thank you Mr. Jones, now I would like to do a brief physical exam on you. Is that okay? Before I start, let me go and wash my hands!

*Do your physical exam, and ask permission before each procedure*
ONce you are done.. always thank the pts.

"Mr. Jones, thank you for letting me do this exam. From the history that I've gathered and the physical exam, it looks like you might have XXXX. But before I can give you an exact diagnosis, I would like to run some test first such as : CBC, TSH, CXR, MRI..etc.. In the mean time I'll change your medications to ..(whatever you need to give or recommend)

Is there any questions you would like to ask? Thank you and I'll see you soon!


If you follow this same exact flow..and know your mnemonics, you will pass for sure! I have helped many friends in the past to pass the CS. Good luck!

dr1704, what is WADE? I've never seen that part of PAMHUGSFOSS. I've heard that I should have done a general ROS (headache, runny nose, cough, chest pain, stomach pain, joint pain, etc.) for every pt and a general PE (eyes, ears, nose, mouth, lungs, cv) before starting the focused PE for every pt. Do you think all of that is necessary for every pt....it seems so time consuming? I did do lungs and CV for every pt.

WADE -> Weight loss/gain, Appetite, Diet, Exercise

You don't have to do all the general ROS. This exam is more of a focus exam.
For the PE, you should do heart and lungs as well. For heart sounds, just listen to two sounds ( Aortic and pulmonary). For Lungs listen to all sounds (bilaterally-total of 6 and compare them.. cause some cases the pts might breath on one side and not the other)

If pts have new Diabetes or comes in with new Hypertension or as a follow up:
You must do full neuro-exam! Cranial nerves, check eyes, neck, DTR, strength, sensation (pin prick) etc..

Now, if the clinical situation says:

"Jon Doe, a 34 yo M with a PMH of Diabetes is here for abdominal pain."
In this case, you don't do the neuro exam. Although you should check his eyes, and legs. THe main concern is his Abdominal pain.. which could be due to gastroparesis. Focus on what he's here for, and not his past medical history.
