Can anyone speak to how big army allows people to do fellowships at the end of their ADSO?

For example, if there is a need and someone is allowed to pursue a fellowship (a big if), how is the ADSO for the renewal contracts designed? I was recently told that for anesthesiology it wasn't 1-for-1 (ie that a 1-year fellowship doesn't lead to a 1-year increased ADSO, but a 4-year ADSO).

I recognize this might be specialty-specific, but does anyone have experience in this?

I believe the correct calculation is six months obligation per six months of fellowship, with a minimum of two years of obligation. So, that one year Pain fellowship incurs a two year service obligation. In the future, if Peds ever does go to to years like the PDs want, then a two year Peds anesthesia fellowship would incur the same two year obligation.

For a civilian sponsored fellowship, I do not know if the two year commitment is doubled to four, or if the original calculation is doubled to one year of payback for each six months of training, leading to the same two years of new ADSO. It's been a long time since I've looked at the actual calculations. @pgg can probably help on that front, since he recently finished a one year civilian sponsored anesthesiology fellowship for the Navy.

6 months ADSO for 6 months fellowship with a minimum of 2 years.

The ADSO for my 1 year full time out service (FTOS) fellowship was 2 years. However, since I had a pre-existing multi year contract when I started fellowship, the FTOS ADSO is served concurrently with the MSP ADSO. End result is no additional time owed.

For those who don't have a multi year contract already in effect on day 1 of the fellowship, the ADSO is sequential.

Yes, and they hosed is big time with the latest iteration of when you can sign a retention bonus (aka “Multi year specialty pay” in the old language). It used to be that you could sign an MSP prior to paying off your HPSP/USU payback and you could make this work in your favor. Now that they have made it so you have to payback all HPSP/USU time before being able to sign a retention bonus this little “loophole” is essentially gone.

Yet another way they are making our pay essentially go in the negative direction compared to inflation over time. (The other one is no changes to the amounts of RB in many years for most specialties). These are technically imaginary losses since we never had the money to begin with. These new changes cost me over $40k in pay that I would have received if they had been implemented just a year later. 🙁

Yeah, it looks like the loophole is effectively closed for most people now. 🙁

Unless fellowship opportunities become so scarce that being competitive for a GMESB nod means completing multiple staff tours post residency. 🙁
