Has anyone applied to a normal/non-military fellowship, particularly in radiology after paying back a 4 year HPSP commitment? How were you treated by the programs?

I have decided not to seek a fellowship directly as a 2 year ADO for a 1 year fellowship is a raw deal IMO.

I cannot speak to radiology specifically, but I have known people that have applied to fellowships following a TOD in the military and they have generally fared very well. My time in the military, while a major pain in the ass most of the time, did teach me some valuable lessons about leadership, adversity, and turd polishing in general. Fellowship and residency directors (at least the ones I've known) happen to like guys that have really 'worked' for a living for a little while.

I think that depend where. If you spent conserable amount time in the operational medicine or non-clinical environment program directors may doubt your clinical ability and prefer third year resident over you. Realistically however this cannot happend as fellowship selection process (composed of board members) heavily favors ones with post utilization tour...😀

I'm pretty sure the OP is referring to civilian fellowships done completely independent of the military.

I swear I wrote a post about this before, but I couldn't find it. The search function here is really weak.
I went to a top rated civilian fellowship after my staff payback. I was heavily recruited everywhere I applied. The fact that I had several years of staff experience including more formal leadership training and experience than most of their mid career faculty, and that I was already board certified, all made me stand head and shoulders above the other applicants. Play up all the unique angles that the military gives you. It's a huge plus. Think about all the BS non clinical stool that you had to do all the time. Head of xxxxx, member of xxxxxx and yyyyyy committee, responsible for training xxxxxx, division chief or department head, battlefield experience, mentoring enlisted guys, innovations that you implemented, etc, etc, etc. I went in there confident that my CV and experience would destroy that of 99.99% of the other fellowship applicants. Who can compete with what you're bringing to the table. They may be looking for a research superstar, and that's a negative in the military, but if the program is that research heavy it's likely to be a poor fit for you anyway. I had my choice of programs, you will too. 👍
