Speaking from point of view of someone applying to nephrology: know thyself and what you want to do with your life. Academic programs like to see someone committeed to an academic career, be able to elaborate on what is it about the specialty that makes you want to do it. Also if you put down research, you need to know that stuff cold including the details and such.
