What percent of orthopods do fellowships? I've heard the number is super high, like above 90%. Is the situation more like you need to have a fellowship to work in any competitive environments like nyc, SF or LA but they are more dispensible in rural arkansas?

Generally speaking you're right. Something around 90% do a fellowship.

If you want to be in a large urban area you pretty much have to do a fellowship. To practice in an academic center in these saturated markets it's not unheard of to do 2 fellowships to get a real niche (ie peds sports)

If you're willing to work in a more rural area, working as a generalist is more feasible.

Personally - I'm doing a short stint as a generalist before going to fellowship (military), but hope to maintain a bit of a generalist practice even afterwards. Doing the fellowship gives me something to fall back on.
