Aww thanks guys! I appreciate it! Feel free to pass it on to new students. I know having a template helps me out from having to think too hard during a patient encounter!

Also add the new mnemonic VINDICATE if you are still learning to formulate good differential diagnoses...

If I were a betting man (which I ain't), psi = psy = psych. e.g. psych history.

No idea what the rabbit is for, though.

No idea about the rabbit. But I did some procrastination googling. I personally would like to side with the Koreans and Japanese on this one: it probably concerns rabbits making rice cakes on the moon.

Oh nuts. I thought this was going to be a contest for the most ridiculous H&P written and put in a chart. Oh well, this is a pretty decent H&P, though the HPI is way too short. And the space for the "Vent" in the physically is way too tiny. I am glad you included a Code status though.
