I knew Jay Loeffler had joined the group. Looks like they finally photoshopped him on the front page. Good for him. He's kind of one of rad onc's longest johnsons is he not. I had a good radiology friend in Florida and he and Loeffler and Joe Namath used to hang out together on private jets and that sort of thing. Now Jay's in Florida makin' real money before he retires. Hope he has some friends at CMS to postpone APM because in that practice he's gonna take a lot of Medicare.

Believes so much in proton therapy (former director at harvard protons as I recall) he now doesn't even need it to practice as a distinguished rad onc.

What do you mean? It’s a totally different practice than he is equipped for. Are you saying community practice is that easy that someone who hasn’t done it for 40 years can easily slip in and be a generalist? Or are you saying that more private practices should hire figurehead old men to put on their website?

Yeah- I wasn’t clear I guess. I would think more people would leave after 5-10 years, but academic retention seems high.

Also, many community practices specialize these days - we certainly did at Banner.
 
It’s rare. Especially after 5 years out. There is a discrimination towards private folks, from what I see. I was told multiple times “we can’t afford you” without actually giving me a number. Also, after not being institutionalized, they may think that private / community folks will be docile enough to fit in.

Would you trust a breast or prostate only guy to go into PP after a couple to several decades in practice? I wouldn't and some of them will tell you in person they wouldn't feel comfortable in a full spectrum practice either

Which is wild. Of course a "breast rad onc" would know how to treat breast. But any other rad onc academic iteration sometimes can't. And a doctor called a radiation oncologist not being able to treat breast would be like a doctor called a surgeon not being able to suture.

Bruce hafty told us in residency he'd feel comfortable doing breast and maybe a glottic and that was about it

With a good dosimetrist and access to the internet, I think it would be hard to find a halfway decent rad onc who couldn’t treat breast. Personally, I’d just prefer not to 🙂
