Does anyone know what requirements, if any, exist in Arizona to operate fluoroscopy? Do you need to register the equipment or take any examinations? Do you need to do badge monitoring? Do you need a rad technologist?

you need to register your machine with the radiation board. you don't need to take an exam. you need to monitor your radiation exposure. you do not need a rad tech to operate your unit.
