Hi guys,

I have had 2 gaps ("leave of absence") in my volunteering at this one big hospital
- one quarter where my schedule left absolutely no 4 hour block because of sports and stuff
- one quarter where I started a new volunteering job at a smaller clinic and could not do both at once

By the time I apply, I should still be volunteering at this place. So should I put start date - Present, and then list gaps?

- Also, does AMCAS ask for total amount of hours so far?
- How do I enter it if I volunteer every other week?

I think you'll be fine with just putting a continuous time. AMCAS asks you to put the average number of hours per week you volunteer there, so you can adjust that number at your discretion. If you want, there is also a box for description and in there you can clarify if you'd like.

I think you're being a bit neurotic here, TBH. Relax. Taking a break from a volunteer position really isn't that big of a deal. Schedules and life circumstances sometimes change. As long as you aren't just holding positions for a few weeks and then changing, which it doesn't sound like you're doing.
