If I do poorly this semester (I'm in my senior year), do I even need to send updates?I know I have to send my final transcript before I matriculate, but do I need to bother sending grades pre-II? For context I had a downward trend and I have lower than average GPA.

Unless a med school asks you to send a grade report, there is no obligation or expectation to advise schools of your fall grades.
