If I take a graduate class, as an undergrad (so it goes on my undergrad record), does it count as an undergrad class on my med school app?

Yes, it will be included in the undergraduate GPA calculation. It would not be a grad level class unless you are accepted to a graduate program and the class is part of working toward that degree.
