Not third year, not intern year, ideally. But look your family and children will be more important to your lifelong meaning and purpose and ongoing legacy than your training, hopefully, so your and spouse’s wishes re that should have priority. You can make it work whenever. Waiting until 2nd year residency/early 30s means you’ll be on the older side to have as many kids as you might want and at more risk for infertility. OTOH you’ll have more money and need less loans or food stamps. I came in medical school with the kids already because that was a sure and nonnegotiable desire for my spouse and I to have children - and we were broke for some years but I’m not sorry to have done it that way. But acknowledging my privilege of test taking superpowers - I did not have to grind super hard to succeed in school or on boards which left more time to be a good enough mom while in school. Had I been on the academic struggle bus before or during med school it might have been different.
