The clinical scenario generally helps provide context as to what we're looking at.

You don't need a clinical scenario; of course it helps to narrow down the differential, but there's enough in the image to tell you what's going on. The easiest/best radiology book i've read is learning radiology; it is about 300 pages and is a quick read.
