Here is an example from my transcript: (I'm a Rutgers student, so maybe another RU student can tell me what they did)

What do I put for "Course Prefix and Number: Enter data EXACTLY AS IT APPEARS ON YOUR TRANSCRIPT (e.g., CHEM 101)"

I dont have a prefix I dont think.. should I just put:
01:355:101 My only question is how do they know what department offered the course? (which matters when classifying things into science and non-science)

    Thanks in advance!

Based on the transcript, your number scheme looks understandable to me. I would follow that.
