I could use a few pointers here...

I understand that the typical thing to do is to itemize the Honors/Awards/Recognitions and elaborate as needed, but what do I put for the prompts (i.e. "Award Date," "Total Hours," etc.) What is the accepted protocol here?

Use the date of graduation or the last date any of them was awarded. Total hours would be zero or the time you sat through award ceremonies. Contact is your school's Registrar.

I saw a similar post a few days ago, but I was hoping to get your take.....

As a transfer student to a 4 year university, do you think it might be useful for us to list the academic honors we recieved at that university? -
1. Cum/Magna/ Summa Cum Laude.
2. Honors Program and Departmental Honors

3. I ask because all though on our primary you will see the yearly trends, there’s no section that shows our cumulative gpa solely at the 4 year school- which is important to convey. This is unless the adcoms look at our actual transcript, which I don’t think Is commonplace.

4. I go to a school with no committee letter and so i’m wondering if it’s best to include a section for the awards stated above^ For ex. ( Graduated Cum Laude from Duke with a 3.77 gpa , with the requirement being 3.72) . This is mainly to showcase how well we did at the 4 year university cumulatively without the CC gpa being factored.

If you have an open space to use, then by all means, go ahead and list these honors. If space is tight, then it's better to include an activity that will have more impact on your candidacy with med schools.

Thanks for your response. The only reason I would want to do this is to showcase how well I performed at the 4 year schools without the CC gpa being factored into it. When you look at my trends you would see the junior and senior GPA's as separate and so if I include the latin honors stated above with a reference to my gpa at the 4 year alone, maybe that would show something?

Q. During junior year (when I first transferred), my gpa at the 4 year was a 3.5x but during my senior year it was a 4.0 ( my cumulative at the 4 year alone was a 3.85 due to the gigantic number of units I took senior year). Now looking at first glance at my trends, my cumulative 4 year gpa alone might not be completely obvious to an adcom- unless they look at my transcript. Even though I have a 3.8x cumulative overall (including cc + 4 year), some adcoms might feel that it was my cc performance that carried me. However, maybe by including the gpa I earned at the 4 year through the various honors, this might help clarify the situation?

Regardless, the senior year 4.0 with its large number of credits will leap out from the AMCAS-provided GPA grid. Well done!

Are 40 units considered a solid amount of units to take in one academic year? 18 of which are BCPM? Or is that about average?
