Cards, GI, pulm, etc etc, will be more difficult the longer you are out of training,especially if you didn't use your hospitalist time to do research and make connections.
It all depends on how you use your time as a hospitalist and what fellowship you are applying. As a hospitalist, you'll have lots of opportinuty to work with Cards, GI, pulm, etc etc. So use that to your advantage.
I went 3 years outpatient pcp and will be starting fellowship in a few months. It was more so what I did 3 years out of residency that helped me secure a fellowship spot.
