How do adcoms make the difference between good, bad, and excellent clinical experience

What is the criteria used to categorize clinical experience by adcoms? Is it hours, the type of experience, how you write, or some other criteria

Mostly hours and exactly what your responsibilities are.

Agree with the above. When and where can matter, too. If it was all from back before you graduated high school, that looks weak if it was "one and done" box checking. If it was all between April and May of the year you applied, that is weak, too.

If your only experience was outside of the US, that's weak, too.
