The answer for the US is easy. No german post grad (i.e. residency/fellowship) training is accepted. Graduating from a German medical school is fine for US training, as long as it's listed in the WHO directry of medical schools
All German university degrees (MD) make you eligible for training in the US, but you take the USMLEs, go through residency training (with very few exceptions), and then apply for your fellowship of interest (if you chose to do so). Having completed a residency/fellowship in Germany may help you get a residency in the US, particularly if you have research experience as well.
